Using resources modeled on palliative care, leaders assist congregants through the stages of merger or closure.  An educational course for leaders participating in the Lay Servant Ministry program of the United Methodist Church was created. Working with terminal patients, the author created exploration sheets to go with each spiritual practice to guide the conversations between the patient and the chaplain in a hospice and palliative care setting. The thesis offers both a theological discussion and a practical model.  The author approached patients with chronic diseases and examined how life-threatening illnesses challenge their view of life and death.  She surveyed and adapted classic ars moriendi literature (Johannes Geiler von Kaysersberg, Martin Luther, Jeremy Taylor) and the views of contemporary theologians speaking to the issues of dying and death.
