The purpose of this study is to show that a consistent and systematic way to approach bioethical dilemmas has evolved in the last quarter century.  The study approaches the discussion of limiting medical treatment from medical, legal, and theological perspectives. Specifically, bioethical dilemmas involving persons with developmental disabilities residing in a state residential facility in California are explored.  The project portion of the study includes a handbook for use with families in considering options regarding limiting medical treatment. It examines specifics of varying hospital organizations, the chaplain's work and qualifications and the diversity of patients and illnesses encountered.  It considers the training and pastoral role of clergy and laity in various facets of hospital ministry.  The project concludes that effective hospital chaplaincy service is an essential Christian ministry. From a literary review of theodicy, the book of Job and crisis management, the author develops numerous hypotheses regarding ministry to critically ill parishioners.  He then surveys and interviews critically ill persons who are both members of selected 'mainline' churches and residents of the Lower Yakima Valley of Washington state. A theological understanding of life, death and ministry was developed from biblical and secular sources.  Divided into six sessions, the educational experience utilized group discussion, audio-visual presentations, simulations, and visitation. Templer's 'Death Anxiety Scale' and subjective self-reporting were used to evaluate the effectiveness of the educational experience. Death comes gradually often taking 10 to 20 years.  Ministering to these patients is a challenge because as speech fails, the patient becomes more frustrated which complicates the relationship to the minister and others.  The longevity of the disease often creates a severe burden for the family as well as the patient physically, emotionally and financially.
