A review of literature on the ars moriendi tradition reveals common themes of hope, compassion, and patience. For the African American community, the Bible continues to be the source for understanding death. Among this community, acceptance of hospice services has been slow. The project was developed to help the African American community embrace hospice care. An end-of-life care workshop that partnered hospice caregivers and African American church leadership was developed. Working with terminal patients, the author created exploration sheets to go with each spiritual practice to guide the conversations between the patient and the chaplain in a hospice and palliative care setting. This study explores the importance of symbols expressed in dreams, what may appear as nonsensical spontaneous comments, and uncharacteristic behaviors of patients at the end of life. The workshops are designed to inform and develop skills interpreting symbols to enhanced palliative care. Staff reports indicate the calming benefits that interpreting symbols extend to the dying patient. The project director also assisted the project participants in the application of Christian theology to their daily hospice service.  In order to achieve this, the project director wrote an eleven-unit biblical reference booklet for the participants and led them in ten related devotions.  The director used tests and conversational questions to evaluate the ministry intervention. With Christ Jesus as host and the source of all Christian hospitality, the church is called to be a 'refuge' for all travelers on life's way. In answering that call, this church hosts a day-care center for children of working mothers, a weekend hospice for youth, a coffee house, an emergency winter night-care shelter and hospitality center, a free medical clinic, a Bread for the City Center offering food, clothing, and counseling, a thrift shop, a telephone hotline, and four hospitality houses offering shelter and rehabilitative aid to homeless people. The project employed the Death Anxiety Scale (Nehrke modification) in a pretest and posttest format to measure changes in the level of fear and anxiety in these patients. Evaluation found that 75 percent of the patients experienced relief of fear and anxiety, while fear and anxiety increased in 19 percent. Research draws from relevant hospice literature, on-site interviews, and responses to questionnaires.  The model highlights four interrelated aspects of life and health: physical, mental, social, and spiritual.  These are relevant elements in the Indian culture and life style. Legislation applying to Medicare-certified hospices required them to employ a 'pastoral or other counselor.'  This stipulation is often overlooked.  1987 research found that generally the social worker fulfills the counseling function.  A survey of Massachusetts Medicare-certified hospices conducted for this project found similarly.  The problem was investigated by the survey, literature review, interview with AAPC's Executive Director, and critique by peer pastoral counselors.
