The project's objective was to present a model for the church to lead efforts to reduce health disparities in underserved communities through collaborations with government and private organizations. Qualitative research was conducted to assess needs and measure project effectiveness, using surveys and interviews. This effort increased health education, outreach, awareness, and identified support resources within the community. Spiritual assessment occurred while providing pastoral care through emotional support and active listening. Considered along with the medical and psychological assessments, a holistic understanding and treatment of the patient was achieved. A greater sense of healing, harmony of body, mind and spirit was achieved. The author facilitated pre- and post-screening clinics and administered questionnaires before and after a twelve-week implementation program to see if there were any change(s) in lifestyle patterns. Participants experienced decrease in hypertension, body mass index and lowered blood glucose levels. The findings showed that persons who get involved in some lifestyle behavioral change can experience positive change. The methodology used was the Proactive Research Method which pro-actively worked toward a transformation of their behavior as noted thorugh qualitative data. It was found that by utilizing several techniques addressed in this project their quality of life was improved as a result of 'spiritual' education and positive 'spiritual' practices. My proposal involved pre-testing subjects to determine their level of knowledge regarding health care issues.  Subsequently, the study conducted a series of seminars designed to increase the participants' knowledge on issues of health care.  Following those seminars, the study evaluated its results by post-testing the participants to determine the extent of acquired knowledge. She compared their lives and ministry with her research on shamans and shamanism across cultures, as well as with the ministerial life and healing ministry of Jesus Christ. Following the method of practical theology, i.e., a praxis-theory-praxis approach, the author began with the experience of Catholic lay chaplains. The four tested congregations exhibited greater interest in faith and health. These congregations determined the most prevalent congregational diseases, stressors, and some previously unknown health issues.
