A qualitative methodology using observation, narratives, face-to-face interviews, and a survey was employed. The findings suggest systemic and organizational issues concerning policies and procedures which include a neglect and lack of positive rewards, reduced staffing and high turnover rates, differing approaches to care planning, constant policy changes, lost leadership, and staff refusal to float. The practicum was conducted from October 21, 2010, to October 21, 2011. Pre-test surveys showed the hospital does well in caring for grieving families at the time of death. However, a desire for more initiatives in the pre- and post-bereavement phase was indicated.
