This qualitative research considered the aforementioned aspects in terms of human formation, deformation, reformation, and transformation. It offers a new model for diagnosis and treatment using the theological framework of Martin Luther's Small Catechism.  A planned questionnaire based on the Ten Commandments is used to collect data for an assessment.  A diagnosis is made that identifies problems with potential for resolution.  Planned interviews based on the Apostles' Creed are used to establish behavioral goals for a plan. Assigned homework based on the Lord's Prayer is used to coordinate the implementation of the plan. This thesis is about the establishment of such a ministry named the Mustard Seed. The change process incorporated can be best articulated by James Brina Quinn's 'Logic Incrementalism.'  Theological study evolved into 'what do you do for the least of them.' It is based on principles of practical theology, using experiences of workers with Servants to Asia's Urban Poor, partnering with squatters in metro-Manila, Philippines.  These experiences are brought into critical correlation with the Christian tradition and the context of the urban poor. Using this information the church developed a two-year program, evaluated it with a follow-up questionnaire, verifying the program had met specific needs.
